Usermin broken for Debian and Redhat?

I've been unable to connect to either Debian or Redhat with the Usermin tool. I get a "cannot connect to server" error page in my browser instead of the log in page (I can get the log in page for Netbsd, but don't have a template account there).

Update: I created a template account on Netbsd and confirm that Usermin seems to work fine on that host.
