On Oct 22 I faxed the required paperwork to efax 772 325 8221 and sent my pmt of 10 credits USD via PayPal, which shows in my records as already processed.

Can you tell me when my shell account will be available?

I have got both your fax and paypal payment.
The problem is that I could not find any template account that would match your application.

I sent you a mail a day after where I informed you about the status and asked you to create an account at http://www.polarhome.com/service/debian/

Once you have created you template account, please send me the account name in order to activate shell access to that account.
